Floridians who use Medicare will have access to six new clinics around the state from Florida Blue and Alignment Healthcare.
The clinics will provide personalized care to Florida Blue Medicare Advantage members through the new BlueMedicare Preferred HMO plan. The Alignment Healthcare Centers are especially designed for comprehensive care of people with chronic conditions and those who need integrated and coordinated care for better health, such as people with diabetes, chronic obstructive pulmonary disease and congestive heart failure. The clinics will also be equipped to handle wound care, pre-operative care and post hospitalization care.

It will improve health outcomes, particularly for those with chronic illness,” Luisa Charbonneau, Florida Blue vice president, Government Markets, said. “Members will continue to see their primary care provider, and then will have Alignment there to support their health whenever it is needed. Two unique features of the clinics are telemedicine options for people who are housebound as well as alerts to Alignment staff triggered by more than 70,000 proprietary algorithms designed to lower the risk of hospitalizations.
